information on the Staywell Rx program.
Members or eligible family members could
save on prescriptions
if they have been diagnosed with Diabetes and/or high blood
pressure.

If you or an eligible family member are diagnosed
with diabetes and/ or high blood pressure, you may need to take
medication to control your condition. It's important to take the
right dose at the right time. Sometimes you may feel you need a
little help to stay on track. Well, help is available - contact your
Heath Advocate Team member.
The first time you call your nurse, you will earn
a prescription discount on eligible medications related to your
condition. To keep your discount, you will need to talk with a nurse
once every 90 days. If you take a generic medication, your eligible
prescription will be free. If you take certain brand-name
prescriptions, you will pay a reduced copay of $15. In both cases,
you must fill your eligible prescription through Medco
Pharmacy-Medco by Mail.
1. Call Healthmatters
at 1-888-227-6598 to confirm your medication is
part of the StayWell Rx program, then enroll. Remember - fill your
eligible prescription through Medco Pharmacy-Medco by Mail
after you call; not before.
2. Have the first phone call with your nurse
and learn about ways to better control your condition.
Fill your eligible
prescriptions for high blood pressure or diabetes medication through
Medco Pharmacy-Medco by Mail after your call. Continue phone calls
with your nurse every 90 days to keep your discount.
